To complete the rebate form you can either:

• click 'view the form' to download a PDF version that you can print and fill out by hand; or
• click 'download the form' for a Word document you can type on and print.

Please make sure you sign your name before you submit the form.

To check a box on a form, just ‘double click’ the desired box and select 'checked'.
